At the end of the session, the facilitator helps participants create workable plans for using what they’ve learned to make positive behavior changes that can improve their health and help them achieve Personal Health Engagement®. Conversation Map sessions are informal, comfortable settings for health education and engagement. Facilitators manage each session to suit the variety of learning styles, literacy levels and time constraints of the participants.
