Conversation Map programs do more than educate. Through face-to-face, small-group sessions, participants learn the health information most relevant to them and relate it to their own lives and experiences. They then create a workable plan for putting that information into practice. How do Conversation Map programs fit into a participating medical practice or institution?

In most cases, a healthcare professional already on staff can be trained to facilitate Conversation Map programs. Healthy Interactions provides the training and Conversation Map education tools at no direct cost to healthcare professionals attending the training or the locations where they work.

Conversation Map program sessions can be coordinated to meet the different needs of different participants. The Conversation Map education tools are portable and easy to use, making it easy to hold sessions in a variety of medical settings or learning environments.
