Cardiovascular Disease (CVD) is the leading cause of death in the United States and in the world. On average, one in three American adults has some form of CVD.* Everyday fitness, weight management and healthy eating can significantly help many people recover from, lower their risk of, or even prevent CVD. But, for many, these healthy lifestyle choices can be challenging to adopt, especially when they run counter to long-held, everyday habits.

What a Conversation Map® Program Can Do

A Conversation Map program engages people in face-to-face discussions about CVD and heart health with like-minded peers and a trained facilitator. Participants are encouraged to relate what they learn to their own lives, and they create workable plans that enable them to take control of their heath and achieve Personal Health Engagement®.

The CVD Conversation Map program can address the topics most relevant to people affected by CVD, including:

  • Learning how to prevent CVD-related events, such as strokes and heart attacks
  • Living with CVD-related health issues, such as high blood pressure
  • Recovering from a heart attack or stroke
  • Preparing for or recovering from heart surgery or a heart transplant
  • Understanding CVD, its various forms and CVD risk factors
  • Strategies and tips for weight management, smart eating, exercise and fitness
  • Raising awareness about the effects of childhood obesity
